To confirm compliance with the selection criteria, the Participating Company must provide the following documents and information electronically:

• A certificate with general information about the company, which includes: legal name, legal address, actual address, date of registration, full name of the director, bank details, contact phone numbers, e-mail, website address, scanned copies of:
  • Copy of the Certificate of State Registration of a Legal Entity or Individual Entrepreneur or an excerpt from the Unified State Register of Legal Entities and Individual Entrepreneurs;
  • Copy of the VAT payer's certificate (if registered as a VAT payer) or the Single Tax Payment Certificate or an Extract from the registers of VAT payers and single tax payers;
  • Copy of the Charter and an excerpt from the minutes of the appointment of the head (for legal entities);
  • Copy of the certificate from the statistics authorities.
• Copy of the balance sheet (form 1, form 2) for the last 2 years.
• The Participant's proposal, containing the technical part (a document reflecting compliance with the technical specifications).
• Documents confirming the quality of goods / services (certificates of conformity, regulatory and technical documentation, a document confirming compliance with the current DSTU and GOST standards).
• Commercial proposal. Appendix 2.
• A list of current clients and partners with whom the cooperation period is 1 year or more (certificate indicating the client's name, contact person (full name, position, contact phone number)).
• Draft contract.
• Additional information confirming the Supplier's experience, solvency, and good reputation.
• Additional information that may be useful in evaluating the Supplier according to the selection criteria.
• Consent of the personal data subject to the processing of their personal data (for all persons whose personal data is contained in the tender proposal). Appendix 2.
