Applicants must provide:

• Company profile with:
  legal name, legal and actual address, registration date, CEO name, bank details, contact phones, email, website

• Scanned copies of:

  • License to trade in pharmaceuticals

  • Certificate of state registration or extract from the Unified State Register

  • VAT payer certificate or simplified tax certificate/extract (if applicable)

  • Company charter and protocol on appointment of the director (for legal entities)

  • Statistical office certificate

  • Balance sheets (Form 1 and Form 2) for the last 2 years

• Technical offer (compliance with specifications)

• Quality documents (certificates of conformity, regulatory documents, DSTU/GOST compliance)

• Commercial proposal

• List of current clients/partners with cooperation terms of 1+ year (with contact details)

• Draft agreement or letter of agreement to contract terms (Appendix 3)

• Additional documents demonstrating supplier’s experience, financial capacity, and reputation

• Consent to personal data processing for all persons mentioned (Appendix 2)

Tender Format

A pool of suppliers will be selected. Specific suppliers for each procurement will be chosen based on price and availability from among the pool participants, upon submission of a specific request.
